During your first visit, you will receive:smile

  • A warm welcome from our team
  • Review of dental & medical history
  • Radiographs and intraoral photographs if indicated
  • Examination of teeth and gums
  • Charting of existing restorations and needed treatment
  • Periodontal charting
  • Cancer screening
  • Patient education
  • Explanation of any treatment needs
  • Cleaning of teeth (if appropriate)

 

After Dr. Leech understands your priorities and concerns and diagnoses your treatment needs, he will discuss his recommendations with you, answering any questions you may have.  Our staff will then go over your financial obligations, and set up any needed appointments.  We want you to have a clear understanding of needed treatment and will be happy to answer any questions you may have.  Should you need help in determining how to pay for dental work, we can guide you in a variety of financial options that we offer.

What to Bring

For your first visit, please be sure to arrive 15 minutes before your appointment time and bring the following information with you:

  • Your completed Dental History and Patient Information forms – you can download those forms here.
  • Your Dental insurance card (if you have Dental insurance)

Important Requests

A parent or guardian must accompany all patients under 18 years of age.

Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.).

Also please let us know if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) OR require medication prior to dental cleanings (i.e antibiotics for pre-med.)

Payment Options

Insurance is welcome and accepted at our practice. We will gladly file all insurance claims on your behalf. We accept all major credit cards, including Visa, MasterCard, American Express, Discover, and offer Patient Financing through CareCredit, Springleaf or Springstone Financial.
